Work Permit

Michigan law requires all workers under age 18 to have a valid work permit before starting a job.

To request a work permit:

1. Choose the correct form below based on the student’s age.
2. Complete the top section of the form.
3. Ask the employer to complete and sign the middle section.
4. Call the office at (231) 386-5153 to schedule an appointment.
5. Bring the completed form to the school for final certification.

School office staff will review, certify, and sign the work permit. The student will receive a copy to provide to their employer.
