Work Permit

Students under the age of 18 must be issued a work permit prior to the start of any employment. To get a work permit, complete the top portion of the appropriate form below based on your age (under 16 or 16-17) and have your new employer complete the middle section. Call Ms. Elizabeth, (231) 386-5153, to set-up an appointment at the school. Bring the form, with top and middle sections complete and signed, to the school for completion. Ms. Elizabeth will certify and sign your document and give you a copy so you can get to work!
